why do you need extra nibs? is it really something that wears out?

Yes they do wear down with constant use... but they last a long time.

Would these nib replacements be compatible with the intuos tablet cth-680? i can't find any nibs that are explicitly compatible with it.

Thank you for the question. Yes, these nibs will work with the CTH680 pen.

someone said "unscrew the base of the pen" for the extra nibs, but mine doesn't seem to have any parts that unscrew. Suggestions?

The nibs are in the pen base. That is the cylindrical base the pen sets in. unscrew the bottom of the base and you will find the extra pen nibs.

Will these nibs work with the Intous 2?

Yes, it's compatible with intous, cintiqs, bamboo, and graphire wacom tablets.
