So will this type of thing not mess up the top of a door?

I really don't know, as the top of the door is much higher than I am and I can't see it. It hasn't moved since I started using it, some time ago, so my guess is that it doesn't, as this is not a heavy-type item. My only concern was that the door would close, and it does. Hope this helps.

Does it come with instructions?

No it does not come with instructions but it is very simple to use. You just put the hooks over a door and adjust the rope by tying a knot to make it shorter or longer per your needs and it is ready to use. I have enjoyed mine very much and would recommend this product.

My door is abut 1 1/2 inches wide, which would be better the metal bracket door attachment or the web strap door attachment? Thanks

Both door attachment go over the top of the door. The metal door bracket is plenty wide enough to fit your door. It really comes down to preference. The metal bracket can stay on the door when you are done with your exercise. This can be helpful if your injury makes it hard to put the pulley up each time. Perfect for shoulder rehab

Used for rehab after shoulder surgery and it worked flawlessly. Fits right over my bedroom door and I just pull a stool over and it is just what I need. I have a heavy arm and it was very sturdy. What a great price for such a vital part of rehab!

Good quality, I like that there is a small piece of foam on the inside of the bracket so it doesn't bang around. Very helpful for my at home physical therapy after shoulder surgery. Easy to use, stays in place. Not painful to use as I follow the directions of my PT. Lightweight.

My spouse uses this as part of her PT injury recovery. It was recommended by her PT as it exercises the shoulder without placing a strain on the joint. She finds it easy to use with smooth, quiet operation. The installation over the top of a door is incredibly easy with the foam pad on the mounting hardware protecting the door's finish.

Well Made

This is perfect. Works exactly as designed. I'm glad I found it. The rope is longer for taller people but all you have to do is pull it through the handle a little more and adjust it to where you want it. Handles are very comfortable too.
